It's that time of the semester again! Here are the mass funding forms for the Spring 2010 semester. After you fill the forms out, print them out, staple them and submit them in the mailbox in the USG office. If you have any questions, please contact VP of Finance, Max Weber at jmw63@case.edu

Mass funding information
Groups should examine each form, and make a determination as to which forms apply to them. Please type all information into the forms!
**Clarification** Please note that despite the fact that groups that were granted recognition in the spring (and thus received mass funding) are not required to fill out the Recognition form in its entirety, these groups are still required to submit answers to the "For Continuing Organizations" questions, located at the bottom of the Recognition Form. Thank You.
